What is a Tracheostomy?
A tracheostomy is a small opening (called a stoma) made at the front of your neck into your windpipe (trachea). A plastic tube is inserted into this hole to help air reach your lungs directly from a ventilator, bypassing your nose and mouth.
The 'Cuff'
A small balloon at the bottom of the tube. When inflated, it keeps the tube in place and prevents saliva from entering your lungs.
Communication
Initially, you won't be able to speak because the cuff blocks air from your vocal cords. Staff will help you use letter boards or mouth words.
Why was this needed?
While a tube through the mouth (endotracheal tube) is used initially, a tracheostomy is better for longer-term recovery because:

| Hygiene: Easier for staff to keep your mouth and lips clean. | |
| Faster Weaning: You can be more awake, allowing you to start breathing exercises sooner. |
Clearing Your Airway (Suction)
The tube can irritate your lungs, causing more mucus. If you aren't strong enough to cough it out, nurses use a suction tube to clear it. This can feel unpleasant and may make you gag momentarily, but it makes breathing much easier.
The Path to Removal
Reducing your dependence on the tube is called 'weaning.' This is a gradual process of strengthening your breathing muscles.
Step 1: Deflating the Cuff
Initially for short periods, air is let out of the balloon to allow you to breathe past the tube and potentially use a speaking valve.
Step 2: Breathing Breaks
You will have short periods off the ventilator to exercise your lungs, slowly increasing the time as you get stronger.
Step 3: Decannulation (Removal)
Once you can cough effectively and swallow safely, the tube is removed. It is a quick, painless process done at the bedside.
Can't Speak?
Ask your nurse for a picture board or a 'speaking valve' if your weaning is far enough along.

- Becomes very red
- Becomes sore or hot
